A 67-year-old male attends your clinic with reduced vision OD. He finds contours difficult to see and letters can also be difficult to distinguish.
The vision has worsened in the last half year and especially in the last month. It's uncertain whether he experiences any metamorphopsias.
He had +1.50D before cataract surgery.
He always had a comitant divergent strabismus.
Many years ago, he underwent surgery for a brain tumour, and ended up with a facial paralysis and lost hearing from his right ear.
BCVA half a year ago was 1.2 (6/5). Today it is 0.3 (6/20) (PH 0.3 [6/20]).
Zeiss HFA VF 24-2 is normal.
Pupils are equal, round and reactive to light. No afferent defect.
Anterior segment is without any indication of his reduced vision. Posterior segment has normal vasculature, optic nerve head and retina.
